Thousands of Irish patients now receive their Wegovy and Mounjaro pens by courier from pharmacies in Northern Ireland, and save €90–€280 every single month doing it. Yet whenever the model comes up, the same three questions follow: Is this actually legal? Is it really the same medicine? And if it's this much cheaper, what's the catch?
Fair questions, all three. When something costs 30–45% less than your local pharmacy, scepticism is the healthy response, the internet is full of "cheap weight loss injections" that are anything but genuine. This article answers the questions properly: the exact legal basis for cross-border supply, the regulation standing behind every step of the chain, why the price gap exists (market structure, not shortcuts), a step-by-step walkthrough of how the process works, and an honest section on who this model does not suit.
The short answers up front. Legal? Yes, with an individual prescription from a registered doctor, dispensed by a licensed pharmacy, for personal use. Same medicine? Identical branded products, Novo Nordisk Wegovy pens and Eli Lilly Mounjaro KwikPens, from the regulated UK/NI supply chain. The catch? There isn't one hiding in the medicine. The saving comes from structural price differences between the UK and Irish pharmaceutical markets. The rest of this article is the evidence.
Why this model exists at all
The GLP-1 revolution created a problem Irish patients feel every month. Semaglutide (Wegovy) and tirzepatide (Mounjaro) are transformative, trial-average weight loss of 15% and 20% respectively, but for weight management they are entirely private in Ireland: no HSE reimbursement, no Drug Payment Scheme cap. At Republic of Ireland pharmacy prices of €285–€595 per month, a year of treatment runs €3,400–€7,000. Meanwhile, ninety minutes up the road, the identical pens sell for £169–£319 (~€196–€369) under UK pricing structures.
A price gap that large, for a medicine people take every week for a year or more, was always going to produce a legitimate delivery model. The only question was whether it would be built properly, with real doctors, real pharmacies and real accountability, or left to the counterfeit market. We built it properly.
The legal position, plainly
Cross-border pharmacy supply between Northern Ireland and the Republic is lawful where three conditions hold: a valid prescription issued for an individually named patient, dispensing by a licensed pharmacy, and supply for that patient's personal use. Every WLI order meets all three.
The regulatory architecture behind it is worth understanding, because it is what separates this from grey-market importing. Post-Brexit arrangements under the Windsor Framework keep Northern Ireland aligned with EU rules for medicines, the same falsified-medicines protections, the same quality standards the EMA applies. That gives NI a unique position on these islands: UK market pricing, EU-consistent medicines regulation, and a land border with the Republic. Every link in the WLI chain has a named regulator: the prescriber is registered with the Irish Medical Council, the medicines are authorised by the EMA and overseen by the HPRA and MHRA, and the dispensing pharmacy is registered with the Pharmaceutical Society of Northern Ireland.
What the law does not permit, and what we would warn any reader against in the strongest terms, is buying prescription injections from websites that require no prescription. That is the falsified-medicines channel, and regulators on both islands have seized fake GLP-1 pens containing everything from insulin (potentially lethal to a non-diabetic) to nothing at all.
How the process works, step by step
- Online consultation, €30 flat. You complete a structured medical questionnaire reviewed by an Irish-registered doctor. The fee is the same for first and repeat consultations, and refunded in full if you are not eligible, you cannot lose money finding out. Eligibility follows the licensed criteria: generally a BMI of 30+, note we do not prescribe to patients with type 2 diabetes or sleep apnoea, these cases are best dealt with by your own GP. - Doctor assessment. History, current medications, contraindication screening (pregnancy, thyroid cancer history, pancreatitis and more). The criteria are published openly: who qualifies.
- Digital prescription. On approval, your prescription transmits electronically to the partner pharmacy in Northern Ireland. No paper, no travel, no waiting room, no border crossing.
- Licensed pharmacy dispensing. The pharmacy dispenses the identical branded pens sold in any UK pharmacy, from the regulated supply chain, with full batch traceability.
- Cold-chain delivery to your door. GLP-1 pens are temperature-sensitive, so they ship in insulated, monitored packaging by tracked courier, typically arriving within days. Straight into the fridge on arrival.
- Ongoing doctor supervision. Dose escalations, side effect management and reviews all happen through the same €30 flat-fee consultations, for as long as you are on treatment.
The numbers: what patients actually save

At the common 5mg–10mg maintenance range the saving is €141–€191 per month; over a twelve-month course that is €1,700–€2,300 kept, after every €30 consultation is counted. For a two-person household both on treatment, the annual difference approaches €4,500.
Why so much cheaper? Three structural reasons, none of them shortcuts: the UK's larger, more competitive GLP-1 market compresses wholesale and retail margins; UK distribution networks for these medicines matured earlier than Ireland's; and NI pharmacies price in sterling under UK frameworks. The product is identical, the market it is dispensed in is not.
Safety: the checklist to run on any provider (including us)

If any provider fails one of those tests, walk away. Cheap becomes catastrophically expensive when the pen is fake or the dose unsupervised.
Who this suits, and who it doesn't
The NI delivery model suits the large majority of eligible patients: people comfortable with online consultations who want genuine medication, real medical oversight and the best legitimate price, delivered discreetly to home or work. Discretion matters more than people admit, no pharmacy counter conversations, plain packaging, complete privacy.
It is not for everyone, and honesty about that builds the trust the model runs on. If you have complex overlapping conditions needing in-person examination, unstable diabetes on insulin, a history that contraindicates GLP-1 treatment, or a BMI below the licensed thresholds, our doctors will decline to prescribe and refund your €30. Roughly one in ten applicants is turned away or redirected to their GP. A service that never says no is not a medical service.

The model is about to matter even more. There is no customs process for the patient and no paperwork beyond your online account. Most patients never think about the border at all, the only visible difference from a local pharmacy is the price and the courier.
Is the medication genuinely identical to what Irish pharmacies sell?
Yes, same manufacturers (Novo Nordisk, Eli Lilly), same branded pens, same batch-controlled regulated supply chain, same patient information leaflet in the box. The UK/NI supply chain operates under falsified-medicines protections consistent with the EU's. What differs is the wholesale and retail pricing structure of the market the pharmacy sits in, that, and nothing else, is the saving.
Why is the consultation only €30 when clinics charge €50–€120?
Deliberate model design: flat, honest pricing with no surprises. €30 first time, €30 every repeat including dose changes, refunded if you're not eligible. We'd rather earn on a long, well-supervised patient relationship at fair medication prices than front-load fees. The €30 also filters out nothing medically, the assessment is the same rigour a €100 consultation buys elsewhere.
