How the Northern Ireland Route Works for Weight Loss Injections: A Step-by-Step Guide

Overview

"The Northern Ireland route" has become shorthand among Irish patients for the way most people in the Republic now access Wegovy and Mounjaro, an Irish-registered doctor prescribes online, a licensed pharmacy in Northern Ireland dispenses, and the pens arrive at your door by cold-chain courier a few days later. It is spoken about constantly and explained properly almost never. People know it exists, know it is cheaper, and know it is legal, but the actual mechanics, the how does this get from a website to my fridge, tend to stay fuzzy.

This guide is the mechanics. This is the step-by-step: what happens at each stage, who is responsible for it, what you personally have to do, how long each part takes, and where the safeguards sit. By the end you will be able to picture the whole journey, from the three-minute form to the injection you give yourself on a Sunday morning, and know exactly what makes it work.

The short version, so the map is clear before we walk it: you complete an online consultation reviewed by an Irish doctor for a flat €30, refunded in full if you are not suitable; on approval your prescription is sent electronically to a licensed Northern Ireland pharmacy; that pharmacy dispenses the identical branded pens and ships them to you in temperature-controlled packaging; and your dose escalation and reviews continue through the same doctor. Six stages, each a regulated actor doing its ordinary job. Here is each one in detail.

Why the route exists

Only so the steps make sense. For weight management, Wegovy (semaglutide) and Mounjaro (tirzepatide) are entirely private in Ireland, no HSE reimbursement, no Drug Payment Scheme cap, so patients pay the full pharmacy price of roughly €285 to €595 a month. The identical pens, from the same regulated manufacturing chain, are dispensed in Northern Ireland for about £169 to £319 (~€196 to €369) under UK market pricing. Northern Ireland's post-Brexit position under the Windsor Framework keeps it aligned with EU medicines rules while pricing in sterling under UK frameworks, which is what makes a legitimate, well-regulated cross-border route possible rather than a grey-market one. That gap, material every month, large over a year-long course, is the entire reason the route was built. Now, how it actually runs.

The route, step by step

Step 1: The online consultation

Everything starts with a medical assessment, because a prescription-only medicine cannot lawfully be supplied without one. You complete a structured clinical questionnaire online: height and weight (for your BMI), medical history, current medications, and screening questions for the contraindications that matter with GLP-1 medicines, pregnancy or breastfeeding, personal or family history of medullary thyroid cancer, a history of pancreatitis, and others.

The fee is €30, flat, the same whether it is your first consultation or a repeat, and refunded in full if the doctor decides you are not suitable. Step 2: The doctor's assessment (usually 2 to 4 hours)

An Irish-registered doctor, verifiable by name on the Irish Medical Council register, reviews what you submitted. They are checking three things: that you meet the licensed eligibility criteria (generally a BMI of 30 or above, or 27 or above with a weight-related condition such as high blood pressure, type 2 diabetes or sleep apnoea); that nothing in your history contraindicates treatment; and, if you are suitable, which medicine fits you best. This is the step people underestimate. It is a real clinical decision, and the answer is sometimes no, roughly one in ten applicants is declined or redirected to their GP, and refunded. A service that approves everyone is not running this step properly. If the doctor needs more information, they will ask before deciding.

Step 3: The digital prescription crosses the border

On approval, your prescription is issued and transmitted electronically to the partner pharmacy in Northern Ireland. This is the moment the route becomes "cross-border", and it is worth noticing how little happens from your side: no paper, no posting, no travel, no waiting room, and, critically, no border crossing by you. The prescription moves; you do not. What makes it lawful is that it is an individual prescription, issued by a registered doctor, for a named patient's personal use, being sent to a licensed pharmacy, every condition the law requires, satisfied before a single pen is dispensed.

Step 4: Licensed dispensing in Northern Ireland (same day to next day)

The Northern Ireland pharmacy, a registered premises with a named superintendent pharmacist, on the UK pharmacy register and overseen by the Pharmaceutical Society of Northern Ireland, dispenses your medicine. And this is the point that most reassures sceptics once they understand it: what comes off that shelf is the identical branded product sold in any UK pharmacy. Novo Nordisk Wegovy pens, Eli Lilly Mounjaro KwikPens, from the regulated wholesale supply chain, with full batch-level traceability back to the manufacturer, and the same patient information leaflet in the box. Under the Windsor Framework the Northern supply chain carries falsified-medicines protections consistent with the EU's. The pharmacy label changes; the medicine does not.

Step 5: Cold-chain delivery to your door

GLP-1 pens are temperature-sensitive, they are stored refrigerated, so they cannot simply be dropped in a padded envelope. They ship in insulated, temperature-monitored packaging by tracked courier, typically arriving within a few days of dispensing. You get tracking; you sign for it; it goes straight into the fridge on arrival. Delivery is to any address in the Republic, home or work, and the packaging is plain, which for a lot of patients matters more than they expect: no pharmacy-counter conversation, no one at the door any the wiser. For the patient, this step is the only physically visible part of the whole route, and it looks like any other tracked parcel.

Step 6: Ongoing supervision and dose escalation (the part that never ends while you're treating)

This is not a sell-and-forget transaction, and the last step is really a continuous one. GLP-1 treatment is deliberately started low and increased in steps over weeks to manage side effects and find your effective dose. Every one of those reviews, dose increases, side-effect management, checking how you are getting on, runs through the same €30 flat-fee consultation with the same clinical oversight. Repeat prescriptions follow the same path as the first: reviewed, issued, transmitted, dispensed, delivered. Supervision is not an optional add-on to the route; it is part of it, because dose management is where both results and safety are actually made.

What the route asks of you and what it doesn't

It is worth being precise about your role, because it is smaller than people assume. What you do: complete an honest medical questionnaire, pay the €30, receive and refrigerate the delivery, inject yourself on schedule, and book reviews as your dose changes. What you never do: travel to Northern Ireland, cross the border, collect anything in person, handle any customs process, or manage any paperwork beyond your online account. There is no customs step for the patient: the prescription travels digitally and the medicine travels by courier as a normal parcel. Many patients tell us they stop thinking about the border entirely after the first order; the only visible differences from a local pharmacy are the price and the courier.

Where the safeguards sit along the route

Because the route touches two jurisdictions, it is fair to ask where the protections are, and the honest answer is that there is a named, accountable regulator at every link. The prescriber is registered with the Irish Medical Council (the team is led by Dr. Conor O'Hanlon, IMC 008009). The medicines are authorised by the EMA and overseen by the HPRA and the MHRA. The dispensing pharmacy is registered with the Pharmaceutical Society of Northern Ireland. Run that same checklist on any provider before you hand over money or medical details: a real assessment before any prescription, a named and registered prescriber, a named and licensed dispensing pharmacy, cold-chain shipping, independent certification, and ongoing reviews rather than a one-off sale. The one warning sign that overrides all of them: a price dramatically below every legitimate pharmacy is not a bargain but a red flag. A legitimate cross-border route saves you money because of market structure; a counterfeit one "saves" you money because there is no medicine, or the wrong one, in the pen.

How the route changes with the Wegovy pill

The route is about to get a new lane. On 15 July 2026 the European Commission approved the Wegovy pill (oral semaglutide) as the first oral GLP-1 for weight management in the EU, a step beyond the CHMP's positive opinion earlier in the year, and confirmation that a tablet version is coming to these islands. A pill removes the injection and, importantly, the cold-chain step from the route, but the same North-South pricing structures that make the injection route work are expected to apply to the tablet too.
